Young persons support worker – South London

Promote Independence & Prepare for Independent Living

We are seeking a young persons support worker for a temporary 3-month position, working 37.5 hours per week on a rota basis (early, mid, late shifts) including 4 out of 8 weekends.

Shifts:

• Early: 8AM – 4PM
• Mid: 12PM – 8PM
• Late: 1:30PM – 10:30PM

Responsibilities:

• Support 7-8 young people with key working sessions and SMART Support Plans.
• Conduct needs and risk assessments.
• Assess and accept suitable referrals.
• Facilitate move-on to appropriate accommodation.
• Help new residents integrate and access local services.
• Support educational and employment opportunities.
• Organize activities to prepare for independence.
• Maximize benefit entitlements and secure project income.
• Prioritize safeguarding and immediate needs.
