A Prison Link Worker is required to work on an initial 3 month contract, with scope to extend, for a service based in West London.
Responsibilities:
As a member of a flexible, multidisciplinary team working in a hybrid model as a prison link worker for the Dependency and Recovery Services, you will be part of a cluster of delivery sites covering primarily custodial settings across London, with some work required in mandated community locations.
Prison link workers will serve as our primary point of contact for all matters pertaining to the prison, including calls, appointment reminders, meet-and-greets, enhanced support for releases with a history of non-engagement, and accompanying People on Probation (POP) to appointments. They will also support engagement and target boroughs with low continuity of care. It will be necessary for you to control the dangers that come with moving into the community, such as treatment dropout, overdose, and mitigations like naloxone.
To increase continuity of care, you will need to comprehend the obstacles and possibilities (pre-release contact, release plan reviews, disengagement strategies, alternate points of contact for the SU upon release (e.g., family/friends), meet and greet services).
Your services will improve treatment access and satisfy the requirements of individuals whose substance use disorders frequently co-occur with other mental illnesses and other complicated needs by offering extra support and assistance.
