Assist street outreach workers in providing support to rough sleepers. Engage with clients, assess their needs, and help them access accommodation and services.

Key Responsibilities:

· Conduct outreach Monday-Friday

· Collaborate with agencies to provide housing and support options

· Advocate for clients’ welfare and healthcare needs

· Manage a caseload, offering guidance until clients are housed

· Maintain accurate case notes on CHAIN and other databases

Mixture of early shifts starting from 06:30-14:30 and 14:00-22:00, with 1 day in the office and 1 day meeting with clients in their home.

Experience as an outreach worker is vital
